Clinical Education Coordinator (RN) - Outreach Healthcare Services
Job Description
Provide community based nursing care and health education classes to identified vulnerable children, adolescents, adults and seniors at various designated sites and locations.
Ensures that outreach healthcare services and education are targeted to disenfranchised, homeless, immigrants, homebound, and the poor. Continuously scans the environmental for unmet community needs and researches, plans, executes and documents project progress. Assists clients in determining eligibility for resources (Medicare, Medicaid, VA, Ryan White, HCH Charity Care, food stamps, etc.) and provides referral information as appropriate. This role necessitates good assessment, critical thinking, prioritization and time management skills. Ability to communicate, self motivate and remain flexible a must.
Qualifications
MINIMUM QUALIFICATIONS
Graduate of an accredited school of professional nursing(BS/N).
Graduate of an accredited school with a Masters degree in Nursing, Education, or related field preferred.
Minimum of two (2) years supervisory experience, preferably with at least one (1) year in community health.
Strong pediatric clinical skills and knowledge required.
Demonstrates a strong commitment of staff training and education.
Demonstrates evidence of leadership, responsibility, dependability and keen knowledge of working with volunteers.
