Job Summary

We are looking for a Home Care Aide to join our team in Sacramento County! You will be directly working with clients, following a one-on-one care plan in clients' homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets clients' needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities

Assist clients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles

Care for clients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care

Maintain records of client care, condition, progress, or problems to report and discuss observations with the supervisor

Build relationships with clients by conversing or reading aloud to help keep them mentally healthy and alert

Medication reminders

Engage clients in physical or occupational therapy and other activities

Qualifications & Requirements

Must have active HCA (Home Care Aide) registration

Must speak Spanish

Must be able to lift at least 50 lbs

Valid California ID

Reliable vehicle with proof of car insurance (required for transporting clients)

Recent TB test (within the past 90 days)
