    Position Overview

    The Home Care Community Liaison and Lead Caregiver is responsible for generating qualified referrals for Assisting Hands of Carlsbad. This leadership role requires a proactive, relationship-driven professional who can build and maintain connections and trust with referral sources, geriatric healthcare professionals, and the broader community. The ideal candidate will have 3+ years in senior services and a marketing/caregiver background. Person must work two weekends per month with flexible schedule the week(s) after.

    Key Responsibilities for Community Liaison (50% of job)

    Develop and implement a plan to connect with social workers, discharge planners, and other industry professionals in hospice agencies, skilled nursing communities, and retirement/assisting living facilities.

    Generate referrals by building and nurturing relationships with physicians, SNFs, assisted living facilities, and other community resources.

    Conduct market analysis to identify trends, competitor activities, and opportunities for growth, using insights to inform marketing strategies and campaigns.

    Lead the creation and execution of community outreach initiatives.

    Represent Assisting Hands at community events, health fairs, and professional networking opportunities to promote services and strengthen the agency’s reputation.

    Collaborate closely with the home care team to align sales efforts with overall organizational goals.

    Work with the National Office on marketing collateral, presentations, and educational materials for referral sources and potential clients.

    Track and analyze the effectiveness of sales activities, adjusting strategies as needed to maximize return on investment.

    Work 30 hours/week in the field (Oceanside and Carlsbad) maintaining and developing relationships.

    Generate a minimum of 5 leads per month with a target of 10.

    Drive weekly 1x1 agenda meeting with supervisor using activity tracking/52 Weeks Marketing template(s).

    Share activity tracking form template with team during our weekly meeting.

    Key Responsibilities for Lead Caregiver (50% of job)

    Supervise and support weekend caregivers.

    Act as the primary point of contact for clients, families, and staff during weekend hours.

    Coordinate and adjust staff schedules to ensure adequate coverage and respond to last-minute call-outs or emergencies.

    Conduct regular check-ins with clients and staff to ensure satisfaction and address any concerns promptly.

    Handle urgent issues, incidents, or complaints in a professional and timely manner, escalating to senior management as necessary.

    Ensure all care services are delivered in accordance with individualized care plans, company policies, and relevant regulations.

    Maintain accurate documentation of weekend activities, incidents, and communications.

    Provide feedback and guidance to staff, fostering a positive and collaborative work environment.

    Participate in on-call duties as required, responding to after-hours emergencies or inquiries.

    When no caregivers can be found to cover a cancelled shift, the Weekend Manager will serve the hours. If the Weekend Manager is unable to cover the shift, they will find another agency who can (BrightStar or Qualicare).

    Closely monitor Elevate phone/text system to respond to immediate and emerging inquiries, including client prospects from various lead sources including Caring.com.

    Compensation

    · $25/hour

    · 3% commission from new client hours billed (paid quarterly) stemming from direct work of Community Liaison
