We are seeking a highly motivated Community Relations Manager / Healthcare Sales Representative to expand our referral network and increase awareness of our services throughout the community.

Position Summary

The Community Relations Manager is responsible for developing strong relationships with healthcare providers, generating referrals, and promoting Executive Home Care’s services throughout the community.

This role is ideal for a relationship-driven professional who enjoys networking, meeting people, and helping families find quality care solutions.

Responsibilities

Business Development

Develop relationships with hospitals, rehabilitation facilities, assisted living communities, physicians, and case managers

Conduct regular in-person visits to referral sources

Identify new partnership opportunities in the community

Attend networking events, health fairs, and senior care events

Referral Growth

Educate healthcare professionals about Executive Home Care services

Convert referrals into active clients through consistent follow-up

Assist families in understanding care options and scheduling assessments

Meet or exceed referral growth targets

Community Representation

Represent Executive Home Care professionally in the healthcare community

Serve as a trusted resource for discharge planners and social workers

Maintain strong relationships with referral partners

Reporting

Track referrals and outreach activities

Maintain referral database and CRM updates

Provide weekly updates on marketing and outreach activity
