Position Summary
Comprehensive Home Health Solutions is seeking a professional, highly organized, and customer-focused Home Care Receptionist & Referral Intake Coordinator to support our growing Home Care operations. This position serves as the first point of contact for clients, referral sources, caregivers, and visitors while coordinating referral intake, scheduling, and daily administrative operations. Working under the direction of the Administrator, this role is essential to ensuring exceptional customer service, timely referral processing, efficient scheduling, and regulatory compliance.
Key Responsibilities
Greet visitors, answer and direct incoming phone calls, respond to inquiries, and provide exceptional customer service to clients, families, caregivers, referral sources, and vendors.
Receive, review, and process Home Care referrals from hospitals, physicians, case managers, assisted living communities, skilled nursing facilities, and other referral sources.
Verify patient demographics, insurance information, authorizations, physician orders (when applicable), and ensure all required documentation is complete.
Coordinate client and caregiver schedules, matching caregivers based on client needs, availability, skills, and geographic location to ensure continuity of care.
Maintain accurate scheduling and referral information within the agency's EMR and scheduling systems (SmartCare, Axxess, AlayaCare, or similar).
Communicate scheduling updates, changes, cancellations, and confirmations promptly with clients, caregivers, clinical staff, and management.
Manage last-minute staffing changes, caregiver call-offs, urgent client requests, and schedule adjustments while maintaining quality service.
Coordinate with clinical leadership and operations staff to support timely admissions, continuity of care, and staffing needs.
Track referrals, maintain intake logs, follow up on missing documentation, and communicate referral status with referral partners.
Maintain organized patient, client, and administrative records while ensuring compliance with HIPAA, agency policies, and applicable regulatory standards.
Support daily office operations, including filing, scanning, data entry, supply management, mail distribution, and other administrative duties.
Assist with quality assurance activities, audit preparation, and other operational projects as assigned.
Cross-train and perform additional administrative or operational duties as needed to support agency goals and business needs.
Qualifications
High school diploma or equivalent required; Associate's or Bachelor's degree preferred.
Minimum of one (1) to two (2) years of experience in home care, home health, healthcare intake, scheduling, or medical office administration preferred.
Experience with EMR or scheduling software such as SmartCare, Axxess, AlayaCare, WellSky, Homecare Homebase, or similar systems preferred.
Working knowledge of insurance verification, referral intake, and healthcare scheduling processes preferred.
Proficient in Microsoft Office Suite (Word, Excel, Outlook).
Excellent organizational, multitasking, customer service, and communication skills.
Strong attention to detail with the ability to prioritize multiple tasks in a fast-paced environment.
Ability to work independently while collaborating effectively with multidisciplinary teams.
Commitment to maintaining confidentiality and compliance with HIPAA and agency policies.
