Job Summary
We are seeking a dedicated and dynamic Home Care Liaison to join our team. In this role, you will be responsible for building and maintaining relationships with healthcare providers, including hospitals, clinics, and nursing homes, to generate referrals for home care services. You will serve as the main point of contact between our agency and healthcare professionals, educating them on the benefits of home care and our services.
Responsibilities
Build Relationships with Healthcare Providers: Develop and maintain strong relationships with hospitals, clinics, nursing homes, and other healthcare facilities to generate referrals for home care services.
Conduct Facility Visits: Visit healthcare facilities regularly to meet with discharge planners, social workers, and other professionals to promote home care services.
Marketing & Outreach: Represent the agency at community events, health fairs, and networking functions to raise awareness of home care services
Inform Healthcare Providers: Share detailed information with healthcare providers about the agency’s services, eligibility requirements, and the advantages of home care options.
Compliance: Ensure that all activities comply with state and federal regulations related to home care services.
Qualifications
Experience in Healthcare or Marketing: At least 3 years of experience in healthcare, home care, or a similar field, with a background in marketing or client acquisition preferred.
Excellent Communication Skills: Strong verbal and written communication skills to effectively engage with healthcare providers, clients, and families.
Driver’s License: Must have a valid driver's license and be willing to travel to different healthcare facilities.
Networking Abilities: Ability to build and maintain relationships with healthcare professionals, referral sources, and community organizations.
Education: A degree in healthcare, marketing, social work, or a related field is preferred but not always required based on experience.
Knowledge of Home Care Services: Understanding of home care services, eligibility criteria, and benefits is essential.
