Overview
The Home Care Liaison plays a vital role in bridging the gap between healthcare facilities and in-home care services. This hybrid position (field + remote) is responsible for managing referrals, coordinating care plans, and cultivating strong relationships with referral sources including hospitals, skilled nursing facilities, and rehabs. The role requires a mix of clinical knowledge, relationship-building, and operational coordination to ensure patients transition seamlessly to home-based care.
Key Responsibilities
- Serve as the primary contact for hospitals, SNFs, and discharge planners regarding new referrals
- Assess patient needs and determine eligibility for service
- Develop and coordinate individualized care plans in collaboration with internal care teams
- Maintain ongoing communication with families, providers, insurers, and partners
- Conduct in-home visits and assessments as needed
- Deliver educational in-services and presentations to healthcare professionals
- Support quality assurance activities, including chart reviews and care plan evaluations
- Provide weekend and on-call coverage for new referrals and coordination as needed
- Maintain accurate records and referral tracking through CRM and EHR systems
Qualifications
- Minimum of 4 years’ experience in healthcare sales, case management, or discharge planning
- Active RN license (or LPN in some jurisdictions, depending on regional guidelines)
- Knowledge of home care operations and post-acute care transitions
- Strong communication, presentation, and relationship-building skills
- Proficiency in CRM platforms (e.g., Salesforce) and Microsoft Office
- Self-motivated, highly organized, and able to work independently in the field
- Valid driver’s license and access to reliable transportation
