Graceful Touch Home Care LLC is seeking a dependable and professional PRN Registered Nurse to conduct in-home assessments and follow-up visits for our clients.

Responsibilities

Conduct in-home health assessments and evaluate client needs.

Monitor vital signs and changes in health status.

Develop and update individualized care plans.

Complete required documentation accurately and promptly.

Communicate concerns or changes to the agency.

Provide follow-up and supervisory visits as assigned.

Position Details

PRN/as-needed schedule

Home visits required

Visits typically take approximately 30 minutes

Minimal paperwork/documentation

Independent travel within assigned service areas required

Qualifications

Current South Carolina RN license

CPR/BLS certification

Valid driver's license and reliable transportation

Strong assessment, communication, and time-management skills

Home care experience preferred
