BrightStar Care is seeking a dedicated Part-Time Physical Therapist Assistant (PTA) to join our growing home health team. This role is ideal for someone who values flexibility, autonomy, and meaningful one-on-one care with clients in their homes. This role does require travel to clients homes, comfort with traveling is necessary. Key Responsibilities:
Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
Improves or minimizes residual physical disabilities of the patient.
Returns the individual to optimum and productive level within patient’s capabilities.
Participates with all other home care personnel in patient care planning.
Takes initial history and makes initial evaluation.
Consults with physicians regarding patient’s progress
Participates in quality assessment and performance improvement activities as assigned.
Prepares clinical and progress notes.
Helps develop the plan of care and revise, as necessary.
Consults with family and agency personnel.
Ability to do extensive bending, lifting and standing on a regular basis.
Requirements:
A graduate of a PTA Program recognized by the American Physical Therapy Association or other National accepted Council or Association
An active license to practice Physical Therapy in MN
A minimum of four (4) years’ experience as a Physical Therapist Assistant, home care experience a plus
Must have criminal background check
Must have current CPR certification
Must have active driver's license and clear driving record
Computer skills.
Ability to exercise initiative and independent judgment
