    Job Summary

    Diligent Home Health Care is hiring a Home Health Scheduler to coordinate caregiver visits, ensure timely admissions, and support seamless in‑home care across the greater Jacksonville service area. You will match caregivers to client needs, manage daily schedules in the EHR, communicate with clinicians and families, and quickly resolve staffing gaps. The ideal candidate is empathetic, highly organized, and possesses excellent customer service skills.

    Responsibilities

    Professional supervise quality, reliable, in-home care.

    Coordinate internal and external staffing needs

    Assign clients to the appropriate clinicians

    Maintain an up-to-date list of on-call and backup staff and use to secure last-minute coverage

    Communicate with caregivers regarding any updates or changes to their schedule

    Communicate with managers and clients to inform them of any schedule or staffing changes

    Assist in the hiring, training, and management of new staff

    Qualifications

    Professional License Certified Nursing Assistant ( CNA)

    1–3 years scheduling or administrative experience (home health, hospice, or post‑acute care preferred).

    Proficient with scheduling software, Outlook, and Excel.

    Strong organization, phone communication, and problem‑solving skills.

    Reliable attendance; ability to work flexible hours and be part of an on‑call rotation.

    High school diploma required; associate’s degree preferred.

    Comfortable with Microsoft Office and other computer programs

    Ability to multitask and prioritize projects

    Excellent customer service and interpersonal skills
