β REQUIRED Experience: β Willingness to provide approximately 12 hours per week of direct client care β Strong organizational and problem-solving skills β Ability to manage multiple staffing priorities in a fast-paced environment β Excellent communication and relationship-building skills
β WITH: β Active STNA, CPR certification, and current TB test π Valid driverβs license with reliable, insured transportation
β PREFERRED Experience: β Social Worker or Nursing degree is a plus β Home care scheduling or caregiver staffing experience β Experience with AxisCare or similar home care software β Background in home care, hospice, assisted living, or healthcare operations β Experience as a Social Worker, Case Manager, Care Coordinator, Discharge Planner, or Patient Advocate

π Caregiver Scheduling & Staffing Coordination: β Manage caregiver schedules using AxisCare software β Ensure full client coverage and prevent missed shifts β Respond to call-offs and urgent staffing needs β Maintain a dependable backup caregiver pool β Monitor overtime and scheduling efficiency
π€ Client Care Coordination: β Match caregivers with clients based on skills, personality, and care needs β Understand client care plans and home environments β Promote client satisfaction and caregiver retention β Act as a liaison between clients, families, caregivers, and healthcare providers
π§ββοΈ Caregiver Support & Field Operations: β Address caregiver and client concerns β Resolve scheduling and care-related issues β Provide field support to caregivers β Maintain strong client relationships and continuity of care
π Staffing & Caregiver Development: β Identify caregiver training needs β Coordinate training with the Hospice Support Nurse β Recognize staffing gaps and assist with hiring needs
πΏ Hospice Coordination: β Assign appropriately trained caregivers to hospice clients β Coordinate with hospice partners as needed
π Field Engagement (Client Care): β Provide approximately 12 hours per week of direct client care β Support clients and caregivers in the field β Assist with coverage when needed
π Compliance & Documentation: β Ensure caregiver documentation meets required standards β Confirm documentation is uploaded in AxisCare β Maintain complete caregiver files β Support compliance with Ohio non-medical home care regulations
