About the Role:

Join Vitalis Healthcare Services LLC as a Home Care Visit Nurse, where you will provide compassionate and personalized care to our clients in Silver Spring, MD. Be part of a dedicated team that values excellence in healthcare and makes a difference in the lives of those we serve.

Responsibilities:

Conduct in-home assessments and develop individualized care plans for clients.

Administer medications and monitor patients’ vital signs and health status.

Provide wound care, catheter care, and other nursing services as needed.

Educate clients and families on health management and care techniques.

Collaborate with multidisciplinary teams to ensure comprehensive care.

Document patient progress and maintain accurate health records.

Respond to emergencies and provide immediate care as required.

Build strong relationships with clients to enhance their comfort and trust.

Requirements:

Current RN license in the state of Maryland.

Minimum of 1 year of nursing experience, preferably in home health or community settings.

Strong assessment, critical thinking, and communication skills.

Ability to work independently and manage time effectively.

Compassionate demeanor with a strong commitment to patient care.

Basic Life Support (BLS) certification required.

Reliable transportation and valid driver's license.

Experience with electronic health records (EHR) preferred.
