    If you are an experienced home health aide who enjoys varied activities that help people lead more independent lives, we’d love to talk to you! We are seeking a kind and compassionate home health aide who has excellent communication and companionship skills to provide basic medical care, light housekeeping duties, personal care, and hygiene tasks so our patients are empowered to live happily at home. You must have 1-3 years of experience in home health implementing care plans, and make your patients’ health and safety your first priority. Responsibilities: • Enable patients to complete personal hygiene tasks like bathing, toileting, shaving, grooming, dressing, etc. • Transport patients as needed to doctor’s offices or other health care services, grocery shopping, or for other errands • Help patients accomplish personal care tasks like scheduling appointments, calling doctors’ offices, and coordinating Medicare and other insurance details • Mitigate issues with medications by helping to open bottles, organizing self-administered dosing, and arranging for refills, and help the patient with meal preparation and planning • Ambulate patients as indicated in the care plan, and help patients complete exercises and physical therapy as needed • Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake. • Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care. • Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space. • Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards. Qualifications: • Pass a full employment background check, drug test, and physical, and possess current CPR certification and first aid training • Physical requirements include being able to lift up to 100 lbs, may be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed • Completion of home health aide training programs from community colleges are acceptable - Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is a plus • 1-3 years of experience as an HHA/ home health aide, personal caregiver, nursing assistant, or personal care aid for a home health agency, at patient homes or care facilities settings is needed • Carry a valid state driver’s license, current auto insurance, and have reliable transportation • Extreme Ownership: Demonstrates accountability, ownership of outcomes, and proactively removes obstacles. • Resourcefulness: Finds effective solutions even with limited resources and independently seeks knowledge. • Passion for Purpose (Big Why): Possesses a strong, genuine dedication to serving and caring for vulnerable adults. • Commitment to Growth: Actively pursues self-improvement and team betterment each day. • Humility: Embraces a solutions-focused attitude, welcoming feedback and putting egos aside for the best outcome. • Reliability: Shows up on time, prepared, and ready to deliver dependable support. • Gratitude: Maintains a positive outlook, seeking and appreciating the good in all situations.
