    Schedule: Full-Time, Salaried Exempt Base Salary: $90,000–$106,000 annually (based on experience) Plus Bonus: $4,000 Quarterly Bonus Potential Travel: Regular travel within assigned specific territory (will not be traveling to other territories) Benefits: Medical, Dental, Vision, PTO, 401(k) + Company Car Provided

    This is not a clinic-bound role. As a Home Dialysis Nurse (HHD/PD), you’ll deliver high-impact, one-on-one care and education directly in patients’ homes — empowering them to safely manage dialysis therapies with confidence and dignity.

    You’ll serve as a trusted clinical expert for patients, caregivers, and providers, ensuring quality outcomes, regulatory compliance, and continuity of care across home hemodialysis and peritoneal dialysis programs. We're hiring in multiple counties, so you will only have to provide support to one of the five (5) above mentioned territories/markets.

    💡 About the Role

    This role blends advanced clinical nursing, patient education, and independent field practice. You’ll assess patient readiness, deliver competency-based training, monitor outcomes, and collaborate closely with nephrologists and interdisciplinary teams.

    You’ll work autonomously in the field while remaining deeply connected to a supportive clinical leadership team. The position requires strong judgment, regulatory knowledge, and comfort operating in patient home environments.

    🔑 Key Responsibilities

    Clinical Assessment & Care Management

    Conduct comprehensive assessments for patients entering and participating in HHD and/or PD programs

    Evaluate patient readiness, clinical stability, and home environment suitability

    Collaborate with nephrologists and interdisciplinary teams to develop and manage individualized Plans of Care

    Monitor outcomes, adherence, and clinical indicators; identify risks and intervene appropriately

    Patient Home Training & Ongoing Support

    Provide in-home training and clinical support for HHD and PD therapies

    Deliver competency-based education on equipment use, infection control, troubleshooting, and emergency procedures

    Adapt teaching to individual learning styles, literacy levels, and caregiver involvement

    Validate patient and caregiver competency to ensure safe, independent home dialysis

    Quality, Safety & Regulatory Compliance

    Ensure compliance with CMS ESRD Conditions for Coverage, CDC, OSHA, and company policies

    Participate in Quality Assessment and Performance Improvement (QAPI) activities

    Support survey readiness, audits, and corrective action initiatives

    Maintain accurate, timely, and compliant clinical documentation

    Staff Training & Clinical Leadership

    Provide training, guidance, and precepting support to nursing staff involved in home dialysis services

    Serve as a subject matter expert for HHD/PD protocols, infection prevention, and patient education standards

    Support onboarding and ongoing competency validation

    Emergency Preparedness & Continuity of Care

    Educate patients and caregivers on emergency preparedness and dialysis continuity

    Assist with patient communication and care coordination during emergency events

    🧠 Nature of Work (Professional Level)

    Requires independent clinical judgment and autonomous field decision-making

    Involves interpretation of complex medical information and professional-level patient education

    Work is non-routine and cannot be fully standardized

    🎓 Required Qualifications

    Active, unrestricted Registered Nurse (RN) license in Florida

    Minimum of 1 year of home dialysis experience (HHD and/or PD)

    Strong knowledge of CMS ESRD Conditions for Coverage

    Excellent clinical assessment, documentation, and patient education skills

    Valid Florida Driver’s License and ability to travel to patient homes

    Current BLS certification

    Experience participating in QAPI or survey readiness activities

    Prior experience training or mentoring nursing staff
