Deliver Expert Care Where It Matters Most — At Home

This is not a clinic-based role. As a Home Dialysis Nurse (HHD/PD), you will deliver high-impact, one-on-one clinical care and education directly in patients’ homes, empowering them to safely and confidently manage home hemodialysis and peritoneal dialysis.

You will serve as a trusted clinical expert for patients, caregivers, and providers, ensuring quality outcomes, regulatory compliance, and continuity of care across the Broward County territory.

    About the Role

This field-based role blends advanced clinical nursing, patient education, and independent practice. You will assess patient readiness, deliver competency-based training, monitor outcomes, and collaborate closely with nephrologists and interdisciplinary teams.

While working autonomously in the field, you’ll remain closely connected to clinical leadership and quality teams. Strong clinical judgment, regulatory knowledge, and comfort working in patient home environments are essential.

    Key Responsibilities

Conduct patient assessments for HHD and PD, including clinical stability and home environment readiness

Provide in-home training and ongoing clinical support for home dialysis therapies

Maintain accurate, timely, and compliant clinical documentation

Participate in QAPI activities, survey readiness, audits, and corrective actions

Support patient communication, care coordination, and dialysis continuity during emergency events

Minimum of 1 year of home dialysis experience in both HHD and PD
