Seeking an energetic, qualified candidate for independent contract to provide psychosocial services to home dialysis patients. Monthly hours: Up to 20 hours a month.
Requirements:
Hold a master's degree in social work with a specialization in clinical practice from a school of social work
At least 2 years of social work experience, preferred
Responsibilities
Responsibilities include but are not limited to:
Psychosocial assessment: conduct comprehensive psychosocial assessments for all patients to identify strengths, needs, and potential barriers to treatment adherence
Care planning: develop and implement individualized care plans in collaboration with the interdisciplinary team to address identify psychosocial needs
Counseling services: provide counseling to patients and their families to assist with adjustment to chronic kidney disease, dialysis treatment, and lifestyle changes.
Resource coordination: assist patients in accessing community resources, financial assistance programs, and support services to enhance their quality of life.
Education: educate patients and their families about coping strategies, treatment options, and self-management skills
Advocacy: advocate for patients' rights and needs within the healthcare system and the community.
Documentation: Maintain accurate and timely documentation of all assessments, interventions, and patient interactions in compliance with facility policies and regulatory requirements
Pay: $40/hour
