    About the Role:

    Extended Family is a premier, fast-growing private duty (non-skilled) home health care agency servicing the Southwest Florida region and is now expanding to Central Florida. This role will have Manatee & Sarasota County as its territory and will have an UNCAPPED monthly commission structure based on recurring client revenue and a quarterly bonus plan based on starts of care. The agency is only accepting applicants with outside sales experience. The Home Care Liaison’s key initiative is to accelerate the agency’s growth by building relationships with referral sources in the agency’s service area. This position will attend sales networking events in the area to represent the agency’s brand and will build close relationships with key referral sources to maximize new client leads and build a business pipeline. The position will report to the agency’s Chief Executive Officer and will also work closely with the Chief Financial Officer to stay aligned regarding the agency’s staffing levels, client needs, weekly KPI targets, and business development strategy. A key aspect of this position will be to implement referral source pipelines to build the agency’s brand and drive revenue to attain continued business growth.

    Responsibilities:

    Develop and maintain relationships with healthcare providers and referral sources.

    Conduct market research to identify new business opportunities and potential clients.

    Present and promote home health services to prospective clients and partners.

    Collaborate with internal teams to ensure high-quality service delivery.

    Track and report on sales metrics and client feedback.

    Attend networking events and community outreach programs to enhance visibility.

    Provide exceptional customer service and support to existing clients.

    Document all sales activity in CRM on a daily basis.

    Stay updated on industry trends and competitor activities.

    Requirements:

    Proven experience in outside sales or account management.

    Strong communication and interpersonal skills.

    Knowledge of home health care services and regulations.

    Ability to work independently and as part of a team.

    Proficient in Microsoft Office Suite and CRM software.

    Strong organizational skills with attention to detail.

    Valid driver's license and reliable transportation.
