· Provide hands-on instruction and support to improve skills in activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
· Teach and assist with personal meal preparation, housekeeping, shopping, and budgeting.
· Support the person’s health and wellness through medication reminders, exercise routines, and nutrition awareness.
· Follow the individual’s Community Support Plan (CSP) and CSP Addendum as developed by the case manager and agency.
· Deliver training based on each person’s assessed needs, preferences, and learning goals.
· Maintain accurate daily documentation of services provided, training outcomes, and progress.
· Report changes in condition, mood, or behavior promptly to the Designated Coordinator or Manager.
· Ensure the person’s home environment is safe, clean, and organized; teach and reinforce safety and emergency procedures.
· Support participation in community and social activities to promote inclusion and independence.
· Promote and protect the individual’s rights and encourage self-advocacy and informed decision-making.
· Collaborate and communicate effectively with team members, supervisors, and case managers.
· Attend staff meetings, required training sessions, and participate in service planning as needed.
