Home Health Aide (HHA) – Home Health (Field Nurse) (Fontana area, CA)

Job Description:

• Provide direct patient care in a home setting, ensuring the comfort and well-being of patients
• Assit patients with personal care (bathing, grooming, dressing)
• Help with mobility and transfers
• Evaluate home environment for hazards or barriers to more independent living.
• Monitor vital signs and report changes to nurse/ supervisor
• Educate and instruct patients, family members, or other patient representatives, in safety
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

Requirements:

• Valid Home Health Aide (HHA) in the state of California
• Minimum 1 year of experience as an HHA in Home Health
• Ability to lift/move patients safely
• Proficient in taking and interpreting vital signs
• Strong communication and interpersonal skills
• Part time position
