    Key Responsibilities

    • Adhere to care plan to ensure safe and competent care for the patient.
    • Assist patients with personal hygiene and support the maintenance of a clean, safe, and healthy environment.
    • Help patients with ambulation as prescribed by the physician and approved or supervised by a clinician or therapist.
    • Support therapy personnel as needed during rehabilitative processes.
    • Encourage patients to achieve maximum independence according to the nursing care plan.
    • Promote mental alertness by engaging patients in activities of interest.
    • Provide emotional and psychological support to patients and household members, fostering trust and confidentiality.
    • Report any changes in the patient’s condition or home environment to the RN or clinical manager.
    • Perform light housekeeping tasks related to creating a safe and comfortable environment, as directed by the clinician.
    • Complete visit reports promptly and accurately within the electronic medical record system.
    • Confirm daily visit schedules to ensure seamless coordination with other staff.
    • Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
    • Understand and adhere to the agency’s process for managing and addressing customer complaints.
    • Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
    • Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.

    Qualifications

    • NYS certified HHA is required
    • Ability to comprehend the basics of personal care, housekeeping, and meal preparation.
    • Must understand and respect patient needs including ethics and confidentiality of care.
    • Must have a valid driver’s license and access to a reliable and insured vehicle.
    • Ability to visit homes that may not be wheelchair accessible.
    • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
    • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
