Job Summary
We are looking for a Home Health Aide to join our team! You will directly work with children and/or adult patients who have disabilities such as severe autism, behavioral cognition conditions, etc., following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
Responsibilities
Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
Supporting behavioral plans and using positive reinforcement techniques when needed
Assisting with transportation to appointments, school, programs, or community activities
Helping clients develop independence and life skills
Responding appropriately to emergencies and following emergency protocols
Respecting client confidentiality, dignity, cultural background, and personal preferences
Supporting educational, recreational, and social activities for children and adults with developmental or physical disabilities
Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
Engage patients in exercises or other activities
Encouraging social interaction and developmental growth
Monitoring behavior and providing age-appropriate support
Qualifications
Graduated from an accredited Home Health Aide program
High School Diploma or GED
One year prior professional experience
Driver’s license required
CPR certification required
