Job Description

We are seeking a full-time PHYSICAL THERAPY ASSISTANT (PTA) for home health patients in Warsaw Indiana and the surrounding area. As a PTA, you will be responsible for tasks such as:

• Providing rehabilitative physical therapy services to patients in their place of residence under the direction of the Physical Therapist based on the physician's plan of care
• Instructing patients on PT and related exercises
• Train and educate patient and caregivers on use of assistive devices
• Completing documentation to support the delivery of PT services
• Reporting changes in the patients' conditions to other members of the care team

Qualifications

• Graduate of an approved Physical Therapy Assistant program
• Current or eligible for state license
• Excellent oral, written communication and interpersonal skills

Preference given to candidates who also have:

• Active membership with the American Physical Therapy Association (not required)
• One (1) year or more experience in home health setting
