Home Health Case Manager RN or LVN In Office Full Time Family Owned
We are a family-owned home health agency in San Antonio with over 30 years of experience, and we are hiring a full-time in-office Case Manager to help coordinate care for patients across the city.

What this job actually looks like
You are the connective tissue of the agency. You keep patients, field nurses, physicians, and the office moving in the same direction.
On a typical day you will:
Coordinate care and schedules for an assigned group of patients
Communicate with field nurses about changes in patient condition
Follow up on physician orders and get them signed and returned
Track recertifications, resumptions of care, and discharges so nothing falls through
Review documentation for completeness and follow up on what's missing
Take calls from patients and families and get them answers
Support the Director of Nursing with care coordination and case conferences
Make occasional field visits when needed (paid separately, plus mileage)
You are not managing ten patients at once in a busy hallway. You are working through a caseload, in an office, with people who pick up the phone when you call.

What we're looking for
Active Texas RN or LVN license — both are welcome to apply
At least 1 year of home health experience preferred. If you don't have home health but you have solid clinical office experience — physician practice, clinic, case management, hospice, or utilization review — we want to hear from you
Organized, detail-oriented, and good at following through without being chased
Comfortable on the phone with patients, families, and physician offices — a good chunk of this job is politely getting people to do what they said they'd do
Comfortable on a computer and in an EMR — WellSky experience is a plus but not required
Current CPR certification
Reliable transportation, valid driver's license, and current auto insurance
Bilingual English/Spanish is a plus

Compensation and schedule
$50,000 per year, or $80,000 per hour, depending on license and experience
Additional per-visit pay plus mileage for any field visits
Full-time, Monday–Friday, in-office
