We are seeking an experienced Registered Nurse (RN) to serve as a Clinical Manager within our Home Health division. This leadership role oversees patient care from admission through discharge while supporting field clinicians and ensuring regulatory compliance.

Position Overview
The Clinical Manager plays a central role in coordinating patient care, supervising staff, and maintaining quality and compliance standards across the agency.
Key Responsibilities
Oversee and coordinate patient care from admission through discharge
Conduct and/or review patient assessments and care plans
Ensure physician orders are obtained, reviewed, and implemented timely
Supervise and support field clinicians (RNs, LPNs, aides, therapists as applicable)
Review clinical documentation for accuracy, completeness, and compliance
Monitor quality assurance, outcomes, and clinical performance indicators
Collaborate with intake, billing, and administrative teams
Assist with audits, surveys, and regulatory readiness
Address clinical issues, incidents, and patient/family concerns as needed
Qualifications
Active Registered Nurse (RN) license in the state of Florida
Minimum 2–3 years of home health experience preferred
Strong knowledge of home health regulations and clinical standards
Experience with care coordination and clinical documentation
Excellent communication, organization, and problem-solving skills
Comfortable using EMR/home health software systems
Preferred Qualifications
Prior experience as a Clinical Care Manager, Case Manager, or Field Supervisor
Familiarity with Medicare and Medicaid requirements
Experience preparing for surveys or audits
