RiverMead is seeking a Temporary Home Health Coordinator to support the health and well-being of our Independent Living residents admitted to Home Health Care. This role is ideal for a compassionate, proactive professional who enjoys working directly with residents and collaborating with interdisciplinary teams as needed to provide exceptional, person-centered care.

Key Responsibilities:

• Conduct resident assessments, interpret tests, and provide treatments.

• Develop individualized care plans and maintain accurate documentation.

• Coordinate care transitions and ensure compliance with policies and regulations.

• Provide counseling and support directly to residents.

• Implement infection control measures and manage virtual urgent care and CPR programs.

• Participate in wellness initiatives, specialized clinics, and quality improvement projects.

Qualifications:

• Degree in nursing from a program accredited by the National League for Nursing.

• Licensed as an RN or LPN by the State of New Hampshire.

• Current CPR certification required.

• Minimum three years of nursing experience; at least one year of home health nursing preferred.

• One year of supervisory experience preferred.

• Demonstrated knowledge and skills to provide care to and communicate with the geriatric population.
