RiverMead is seeking a Temporary Home Health Coordinator to support the health and well-being of our Independent Living residents admitted to Home Health Care. This role is ideal for a compassionate, proactive professional who enjoys working directly with residents and collaborating with interdisciplinary teams as needed to provide exceptional, person-centered care.
Key Responsibilities:
Conduct resident assessments, interpret tests, and provide treatments.
Develop individualized care plans and maintain accurate documentation.
Coordinate care transitions and ensure compliance with policies and regulations.
Provide counseling and support directly to residents.
Implement infection control measures and manage virtual urgent care and CPR programs.
Participate in wellness initiatives, specialized clinics, and quality improvement projects.
Qualifications:
Degree in nursing from a program accredited by the National League for Nursing.
Licensed as an RN or LPN by the State of New Hampshire.
Current CPR certification required.
Minimum three years of nursing experience; at least one year of home health nursing preferred.
One year of supervisory experience preferred.
Demonstrated knowledge and skills to provide care to and communicate with the geriatric population.
