Home Health Director of Nursing (RN) – $120K+ | Full Benefits | 600+ Patient Agency

A growing Home Health agency serving 600+ patients is seeking an experienced Registered Nurse (RN) to serve as Director of Nursing (DON) and oversee all clinical operations, nursing staff, and patient care services.
This leadership role is responsible for ensuring high-quality patient outcomes, strong clinical oversight, regulatory compliance, and operational excellence within a patient-centered home health environment.
Key Responsibilities
- Oversee all clinical operations and patient care services for a high-volume home health agency
- Supervise and support home health nurses, case managers, and clinical staff
- Ensure compliance with Medicare and Medicaid home health regulations
- Review and ensure accuracy of OASIS documentation including SOC, ROC, and Recertifications
- Oversee clinician documentation and weekly nursing notes for quality assurance
- Ensure treatment plans are signed, returned, and implemented in a timely manner
- Collaborate with physicians, case managers, and interdisciplinary care teams
- Provide clinical guidance and support to RN Case Managers and home health staff
- Ensure staff maintain continuing education and competency standards
- Lead and oversee the Quality Assurance Performance Improvement (QAPI) program
- Coordinate case conferences for complex, high-risk, and high-volume patients
- Support development and implementation of clinical policies and procedures
Minimum Qualifications
- Graduate of an accredited school of professional nursing
- Active Registered Nurse (RN) license – State of Michigan
- Minimum 2 years of home health management or administrative experience
- Strong knowledge of Medicare and Medicaid home health regulations
- Experience supervising home health clinical staff
- Familiarity with OASIS documentation and home health compliance standards
- Experience with Kinnser / WellSky software preferred
- Valid driver’s license and reliable transportation
