    Job Title: Home Health Field RN (PRN)

    Job Description:

    We are looking for a compassionate and dedicated Home Health Field Registered Nurse (RN) to join our team on a PRN (pro re nata, or "as needed") basis. As a Home Health Field RN, you will play a vital role in providing high-quality healthcare services to our patients in the comfort of their homes. This position requires excellent clinical skills, a patient-centered approach, and the ability to work independently while being a part of a collaborative team.

    Key Responsibilities:

    - Conduct comprehensive assessments of patients' health conditions and develop personalized care plans in coordination with the healthcare team.
    - Administer medications, wound care, and other treatments as prescribed by the attending physician.
    - Monitor and document patients' progress, updating care plans as necessary to ensure optimal outcomes.
    - Oversee patients receiving Certified Nurse Aide (CNA) services and provide CNA supervision.
    - Oversee IHSS patients and validate skills for Health Maintenance Attendants (HMAs).
    - Educate patients and their families on disease management, medication administration, and other healthcare needs to promote awareness and self-care.
    - Coordinate with other healthcare professionals, including physicians, occupational and physical therapists, to provide a multidisciplinary approach to patient care.
    - Maintain accurate and timely electronic health records in compliance with organizational policies and procedures.
    - Provide emotional support and guidance to patients and family members to enhance patient experience and satisfaction.

    Qualifications:

    - Current and unrestricted Registered Nurse (RN) license in the state of practice.
    - Minimum of one year of experience in home health nursing or related clinical field preferred.
    - Proficiency in using electronic health records and mobile communication devices.
    - Strong organizational skills with the ability to manage multiple patients and priorities effectively.
    - Excellent communication and interpersonal skills to build rapport with patients and families.
    - Compassionate approach to patient care, with a focus on quality, patient safety, and satisfaction.
    - Valid driver's license and reliable transportation for commuting to patient homes.

    - PRN: This position offers flexible scheduling where you will work on an as-needed basis, allowing you to balance professional and personal commitments.
