Job Summary
The Home Health Intake Lead oversees the intake and eligibility process for a home health agency, ensuring timely referral processing, accurate insurance verification, and compliant clinical review. This role works closely with operations and clinical leadership to support efficient admissions and high-quality patient care.
Key Responsibilities
Intake & Eligibility
Oversee daily intake and eligibility operations.
Review and prioritize referrals from hospitals, physicians, and facilities.
Verify insurance benefits, eligibility, and authorization requirements.
Ensure accurate EMR documentation and timely follow-up to prevent SOC delays.
Clinical Review & Compliance
Conduct clinical review to determine appropriateness for home health services.
Review orders and documentation for Medicare and payer compliance.
Collaborate with RNs and clinical leadership on admissions and care planning.
Identify and escalate compliance or eligibility concerns as needed.
Collaboration & Leadership
Partner with the Operations Manager to improve intake workflows and efficiency.
Coordinate with clinical, billing, and sales teams to ensure smooth admissions.
Train, support, and oversee intake staff performance.
Quality & Reporting
Monitor intake metrics, referral conversion, authorizations, and SOC timelines.
Support audits, reporting, and process improvement initiatives.
Maintain high standards of customer service with referral sources and families.
Qualifications
Required
2–3 years of home health experience (intake, admissions, or clinical review).
Knowledge of Medicare home health guidelines and payer requirements.
Experience with EMR systems and insurance verification.
Strong organizational and communication skills.
Preferred
Active Texas LVN license.
Prior intake leadership or supervisory experience.
Medicare Advantage and commercial payer authorization knowledge.
