Licensed Vocational Nurse (LVN) – Home Health Marin County, California Full-Time | Part-Time | Per Diem

We are seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to join our growing Home Health team serving patients throughout Marin County. Position Summary The Home Health LVN provides skilled nursing care under the direction of a Registered Nurse and physician, ensuring high-quality patient care in accordance with agency policies and state regulations. The LVN works closely with patients, families, and the interdisciplinary team to promote positive health outcomes and patient independence.

Responsibilities

Perform skilled nursing visits in patients' homes

Monitor and report changes in patient condition

Administer medications and treatments as ordered

Provide patient and caregiver education

Perform wound care and other skilled nursing procedures

Complete accurate and timely clinical documentation

Coordinate care with physicians and interdisciplinary team members

Qualifications

Current California LVN License in good standing

Current BLS Certification

Valid California Driver's License and reliable transportation

Strong clinical, communication, and organizational skills

Home Health experience preferred, but not required
