We are looking for a shift care Licensed Vocational Nurse (LVN) to care for a 61 year old with brain injury, Total care - Trach and G-Tube feeding.

Shifts to be covered:

Every other Saturday 7am - 7pm(12hr shift)

Responsibilities and Duties:

Assess patients’ condition during shift and chart observations

Perform evaluation tasks, including vital signs

Administer medications/treatments as ordered

G-Tube care

Trach Care

Provide appropriate timely feedback to RN Case Manager and other professionals

Qualifications and Skills:

CA Vocational Nurse license required

1+ yr experience in Home Health

Recent TB Skin Test or Chest X-Ray

Physical examination within 6 months

Health Care Provider CPR Certification

Comfortable with Feeding Tube/Trach/Vent (agency willing to train)

Ability to travel (locally) with reliable transportation
