About the Role:
Join Bellestar Home Health Care Inc as a Home Health Liaison and play a vital role in connecting patients with essential home health services. We are dedicated to providing compassionate care and improving the quality of life for our clients in Roseville, CA.
Responsibilities:
Act as the primary point of contact between patients, families, and healthcare providers.
Conduct assessments to determine patient needs and coordinate appropriate care plans.
Facilitate communication and collaboration between multidisciplinary teams.
Provide education and support to patients and their families regarding home health services.
Monitor patient progress and adjust care plans as needed.
Maintain accurate and timely documentation of patient interactions and care plans.
Participate in community outreach to promote home health services.
Ensure compliance with healthcare regulations and company policies.
Requirements:
Minimum of 2 years of experience in home health, case management, or healthcare liaison roles.
Valid California driver’s license and reliable transportation.
Strong communication and interpersonal skills to build relationships.
Ability to work independently and manage multiple priorities.
Knowledge of healthcare regulations and home health services.
Compassionate demeanor with a passion for helping others.
Proficient in using electronic health records (EHR) systems.
