About the Role:

Able Home Healthcare in San Diego, CA is looking for a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home health team. Responsibilities:

Provide skilled nursing care to home health patients in accordance with physician-approved care plans

Administer medications, wound care, injections, and other treatments as prescribed

Monitor and document patient vital signs, health status, and progress toward care goals

Communicate effectively with patients, families, and interdisciplinary care team members

Educate patients and caregivers on disease management, medications, and home safety

Maintain accurate and timely clinical documentation in compliance with home health regulations

Identify changes in patient condition and escalate concerns to the supervising RN or physician as needed

Requirements:

Active California Licensed Vocational Nurse (LVN) license in good standing

Previous home health, hospice, or community health nursing experience preferred

Current CPR/BLS certification required

Valid California driver's license and reliable personal transportation

Strong clinical assessment and critical thinking skills

Proficiency with electronic health records (EHR) and clinical documentation

Compassionate, self-motivated, and able to work independently in a field-based setting
