Qualifications:

• LPN License – Current and valid license from the Arizona State Board of Nursing.
• Home Health - experience is required, Wellsky (Kinnser) exp preferred.
• Driver’s License – A valid Arizona driver’s license.
• Reliable Transportation – A dependable personal vehicle with auto insurance.
• CPR Certification – Must have current certification.
• TB Clearance – A valid TB clearance health card (or the ability to obtain one) meeting federal and state requirements.
• Education – Graduation from an accredited Licensed Practical Nurse (LPN) program.
