A Home Health Licensed Vocational Nurse (LVN) is needed in Houston, TX.

Salary and Benefits

• Salary range: $55,000 – $60,000
• Health insurance ($250 reimbursement per pay period)

Responsibilities

• Monday–Friday schedule
• Patient census: 20+ and growing
• EMR: Kinnser / WellSky
• Manage a caseload of home health patients
• Complete and coordinate documentation and patient assessments (within LVN scope)
• Assist with admissions, plan of care coordination, and patient care procedures
• Collaborate with interdisciplinary team members to ensure quality outcomes
• Provide direct nursing care and case management support to patients and families

Requirements

• Clear and active Texas LVN license
• Medicare home health experience
• Familiarity with OASIS documentation
• Strong clinical and organizational skills
