This position educates referral sources, strengthens community partnerships, and helps ensure timely, appropriate patient transitions into home health services.

Qualifications

2+ years home health marketing

Medicare home health experience

Existing Inland Empire relationships preferred

Reliable transportation

Excellent communication skills
