Salary and Benefits

• Base salary of $130,000
• 10% annual performance bonus (based on base salary)
• Total earning potential of $143,000+

Responsibilities

• Deliver high-quality primary care directly in patients’ homes
• Provide ongoing care for geriatric patients with chronic and complex conditions
• Coordinate closely with Medical Assistants and interdisciplinary care teams
• Maintain accurate and timely documentation of visits and care plans
• Assist in scheduling and identifying patients for home-based visits
• Participate in quality improvement activities and patient care planning
• Travel within a defined geographic region using personal transportation

Requirements

• Nurse Practitioner degree from an accredited program
• Active, unrestricted Nurse Practitioner license in Missouri
• Minimum 1 year of experience in geriatrics, internal medicine, or family medicine
• BLS/ACLS certification
• Proficient in managing chronic conditions in a home-based setting
• Able to lift and transport up to 20 lbs. of equipment
• Excellent clinical decision-making and communication skills
• Reliable vehicle and valid driver’s license
