About the Role:

Talin Home Healthcare LLC is looking for a compassionate and dedicated Home Health Nurse to serve patients in the Culpepper area. Responsibilities:

Provide skilled nursing care to home health patients in the Culpepper service area

Conduct comprehensive patient assessments and develop individualized care plans

Administer medications, wound care, IV therapy, and other clinical treatments as ordered

Monitor and document patient progress, vital signs, and changes in condition

Coordinate care with physicians, therapists, and other healthcare team members

Educate patients and family members on disease management, medications, and home safety

Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

Active RN license in the state of Virginia

Previous home health, community health, or clinical nursing experience preferred

Strong assessment, critical thinking, and clinical decision-making skills

Excellent communication and patient education abilities

Reliable transportation and valid driver's license for travel within the Culpepper area

Ability to work independently and manage a patient caseload efficiently

Compassionate, patient-centered approach to care

Must be willing to work at least 2 days a week with 1 of those days be either Monday or Tuesday.
