About the Role:

Join CARE ESSENTIALS HOME HEALTH as a Home Health Occupational Therapist and make a significant impact on the lives of our patients in Riverside, CA. In this fulfilling role, you will provide personalized therapy services to enhance the quality of life for individuals in their homes.

Responsibilities:

Conduct comprehensive evaluations to assess patients' occupational therapy needs.

Develop and implement individualized treatment plans to improve patients' daily living skills.

Provide hands-on therapy and support to patients to achieve their rehabilitation goals.

Educate patients and families on home modifications and adaptive equipment use.

Monitor patient progress and adjust treatment plans as necessary.

Collaborate with a multidisciplinary team to ensure holistic patient care.

Document patient care and treatment outcomes in compliance with healthcare regulations.

Maintain a compassionate and professional demeanor while building rapport with patients.

Requirements:

Master's or Doctoral degree in Occupational Therapy from an accredited program.

Current state licensure as an Occupational Therapist in California.

Previous experience in home health or outpatient therapy preferred.

Strong communication and interpersonal skills to engage with patients, families and staff.

Ability to work independently and as part of a collaborative team.

Knowledge of evidence-based practices in occupational therapy.

Compassionate and patient-centered approach to care.

Valid driver's license and reliable transportation for home visits.
