We are looking for a per diem Occupational Therapist with recent Home Health experience contractor or employee to visit patients in their homes in the Riverside and Moreno Valley area.

Responsibilities and Duties:

Assess patient's condition under a physicians orders during every visit and chart observations

Perform evaluations, Discharges, Recerts and possible follow up visits

Follow prescribed treatments, following plan of care and assess progress of healing

Provide appropriate timely feedback to physician and other professionals

Charting and documenting on EMR system
