Home Health Occupational Therapist (OT)
A Home Health Occupational Therapist is needed in Brooklyn, NY.

Salary and Benefits
- Salary range: $100,000 – $120,000 annually
- 25 PTO days in year one
- 6 paid holidays

Responsibilities
- Provide in-home occupational therapy following the physician’s care plan
- Achieve 30 points per week
- Assess, treat, and develop individualized care plans to improve patient’s ability to perform daily activities
- Monitor patient progress, document outcomes, and adjust treatment as needed
- Educate patients and caregivers on home exercises, adaptations, and assistive devices
- Collaborate with the care team to ensure comprehensive patient care
- OASIS experience preferred
Requirements
- Current Occupational Therapy Licensure in New York State
- Graduate of an accredited OT program
- Proven experience in home health or occupational therapy
- Strong communication and interpersonal skills
- Knowledge of current treatment practices and clinical expertise
