About the Role:
Able Home Healthcare is looking for a passionate and skilled Home Health Occupational Therapist to join our growing team in San Diego, CA. Responsibilities:
Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans for home health patients
Provide skilled OT interventions focused on ADLs, fine motor skills, cognitive rehabilitation, and functional independence
Monitor and document patient progress, adjusting care plans as clinically appropriate
Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated patient care
Educate patients and caregivers on therapeutic techniques, adaptive equipment, and home safety modifications
Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
Manage a caseload of home visits throughout the San Diego area
Requirements:
Master's or Doctoral degree in Occupational Therapy from an accredited program
Current California OT license in good standing
NBCOT certification required
Home health experience preferred; new grads with clinical fieldwork in home health encouraged to apply
Valid driver's license, reliable transportation, and current auto insurance
Strong knowledge of Medicare home health guidelines and documentation requirements
Excellent communication, time management, and organizational skills
Compassionate, self-motivated, and comfortable working independently in the field
