    Job Description:

    Qualifications

    Graduate of an approved school of professional nursing and currently licensed in the State of Texas as an RN.

    Minimum of three (3) years of nursing experience; at least one (1) year of home health experience preferred.

    Acceptance of the philosophy and goals of the Agency.

    Ability to exercise initiative, independent judgment, and strong leadership skills.

    Compliance with all applicable rules and regulations governing the scope of RN practice in the State of Texas.

    Must be in good standing with the Texas Board of Nursing and maintain continuing education requirements.

    Responsibilities

    Overseeing and supervising nursing staff, including orientation, development, and performance evaluations.

    Assisting in recruiting, hiring, evaluating, and, if necessary, terminating nursing personnel.

    Coordinating patient care and collaborating with clinicians, case managers, and physicians to ensure continuity and quality of care.

    Developing, implementing, and maintaining policies and procedures compliant with Medicare, Medicaid, state, and local laws.

    Assisting in planning and coordinating interdisciplinary team services.

    Monitoring clinical documentation, medical records, and patient plans of care for compliance and quality.

    Participating in Quality Assurance Performance Improvement (QAPI) initiatives, studies, and committee meetings.

    Serving as a resource and role model to all staff, fostering a cooperative and professional work environment.

    Consulting with the Director of Clinical Services/Administrator to determine staffing patterns, optimize resource utilization, and ensure patient care objectives are met.

    Assisting with staff meetings, patient care conferences, and maintaining proper documentation.

    Observing confidentiality and safeguarding all patient-related information in accordance with HIPAA standards.

    Monitoring equipment usage and ensuring appropriate resources and supplies are available.

    Monitoring infection control programs, adverse events, and implementing corrective actions as needed.

    Assisting with disciplinary actions and guiding staff in professional development.

    Participating in community education and outreach programs.

    Performing nursing functions during staff shortages or emergencies.

    Being available to share on-call duties with nursing personnel as needed.

    Investigating and reporting any incidents, accidents, or unusual occurrences to the Director of Clinical Services/Administrator.

    Attending relevant continuing education programs and sharing new information with staff to maintain high standards of care.
