Physical Therapist (PT) / Physical Therapist Assistant (PTA)

Responsibilities:

Evaluate patients' mobility, strength, and functional abilities

Develop and implement individualized treatment plans in collaboration with physicians

Provide skilled therapy interventions and adjust treatment plans as needed

Monitor patient progress and outcomes

Educate patients and caregivers on therapy goals and home exercise programs

Complete accurate and timely documentation (Axxess experience preferred)

PT/PTA Requirements:

Valid Oklahoma PT or PTA license

CPR Certification

Reliable transportation and valid Oklahoma driver's license

Strong communication and documentation skills

Home health experience
