Deliver Personalized Care

• Evaluate patient mobility and create individualized treatment plans
• Use evidence-based techniques to improve strength, function, and independence
• Track progress and adjust care plans to ensure real results

Support & Empower Patients

• Educate patients and families throughout the recovery process
• Build confidence and encourage long-term wellness
• Celebrate progress at every stage of recovery

Collaborate with Care Teams

• Work closely with nurses, therapists, physicians, and caregivers
• Maintain clear documentation and communication for continuity of care

What You Bring

• Active Arizona Physical Therapist license
• Home health experience preferred (but not required)
• Strong clinical skills and time management
• Excellent communication and patient-focused mindset
• Reliable transportation
