About the Role:
Angelicare Home Health Inc is looking for a passionate and dedicated Home Health Physical Therapist to join our growing team in Temecula, CA. Responsibilities:
Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans
Provide skilled PT interventions to improve patient mobility, strength, balance, and functional independence
Monitor and document patient progress, adjusting care plans as clinically appropriate
Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
Collaborate with the interdisciplinary care team including nurses, OTs, and physicians to ensure coordinated care
Maintain accurate and timely clinical documentation in compliance with Medicare and state regulations
Participate in care conferences and contribute to discharge planning
Requirements:
Active California Physical Therapist (PT) license in good standing
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Home health or outpatient PT experience preferred
Knowledge of Medicare home health guidelines and OASIS documentation
Valid driver's license, reliable transportation, and current auto insurance
Strong clinical assessment, critical thinking, and patient communication skills
Ability to work independently and manage a flexible caseload in the field
