About the Role:

Able Home Healthcare is looking for a compassionate and dedicated Home Health Physical Therapist Assistant to join our growing team in San Diego, CA. Responsibilities:

Implement physical therapy treatment plans established by the supervising Physical Therapist

Provide in-home therapeutic exercises, gait training, and functional mobility interventions

Monitor and document patient progress, reporting changes in condition to the supervising PT

Educate patients and family members on home exercise programs and safe mobility techniques

Coordinate care with the interdisciplinary home health team to ensure continuity of care

Maintain accurate and timely clinical documentation in compliance with Medicare and agency standards

Travel to patient homes throughout the San Diego area on a scheduled caseload

Requirements:

Associate's or Bachelor's degree from an accredited Physical Therapist Assistant program

Active California PTA license in good standing

Previous home health or rehabilitation experience preferred

Strong knowledge of Medicare home health guidelines and documentation requirements

Excellent communication, time management, and organizational skills

Reliable transportation, valid driver's license, and auto insurance

Compassionate, self-motivated, and able to work independently in a field-based setting
