    Job Summary

    We are a home health care agency seeking a licensed Physical Therapist (PT) to join our team! In this role, you will work closely with patients in their homes, providing comprehensive physical therapy services to support their recovery and improve functional mobility.

    As a Physical Therapist, you will be responsible for evaluating patients, developing individualized treatment plans, and implementing evidence-based interventions tailored to each patient’s needs. You will guide, motivate, and educate patients and their families on therapeutic exercises, mobility strategies, and safety practices to promote independence and overall well-being.

    You will monitor patient progress, adjust care plans as necessary, and collaborate with physicians, caregivers, and interdisciplinary team members to ensure optimal outcomes. Additional responsibilities include maintaining accurate and timely documentation, reporting any changes in patients' conditions, and ensuring that all equipment and treatment environments meet safety and cleanliness standards.

    The ideal candidate is able to work independently, demonstrates strong clinical judgment, communicates effectively with patients and team members, and maintains a positive, compassionate attitude that encourages patient engagement and recovery. A strong understanding of physical therapy principles, current best practices, and regulatory guidelines is essential.

    Responsibilities

    Perform comprehensive patient evaluations and develop individualized, evidence-based treatment plans based on clinical findings and physician orders

    Implement and oversee therapeutic interventions, ensuring proper technique, patient safety, and progression of care

    Regularly assess patient progress, modify treatment plans as needed, and establish appropriate discharge plans

    Document all evaluations, treatments, progress notes, and outcomes accurately and in a timely manner in accordance with regulatory standards

    Communicate and report any significant clinical changes to physicians and the interdisciplinary care team

    Educate patients and caregivers on diagnosis, prognosis, home exercise programs, and safe use of assistive devices such as walkers, canes, and wheelchairs

    Provide guidance, motivation, and support to patients to improve functional independence and quality of life

    Collaborate with other healthcare professionals to coordinate comprehensive patient care

    Ensure all equipment is properly maintained and that the treatment environment is clean, safe, and compliant with health and safety regulations

    Qualifications

    Current state licensing for physical therapy is required

    Current CPR

    Current up-to-date yearly physical and vaccines.

    Current active driver's license, insurance, and vehicle

    Excellent communication and interpersonal skills

    The ability to create a positive, nurturing environment

    Familiarity with basic computer skills

    The ability to work well and make decisions in a high-stress environment
