What You’ll Be Doing
Help patients restore function, improve mobility, reduce pain, and prevent disability
Support patients in achieving the highest possible level of physical independence
Participate in interdisciplinary care planning with the full home care team
Perform comprehensive initial evaluations, OASIS Start of Cares (SOCs), and develop individualized plans of care
Administer prescribed physical therapy treatments and skilled interventions
Conduct 30‑day reassessments, adjust care plans, and update documentation as needed
Provide instruction to the patient and family on safe mobility, strengthening, and home programs
Supervise up to two Physical Therapist Assistants (PTAs) in accordance with Nebraska state guidelines and agency policies
What We’re Looking For
A genuine passion for helping others live healthier, more independent lives
Graduate of an accredited Physical Therapy program
Current Nebraska Physical Therapy license in good standing
Current or eligible NPTE certification
At least one year of experience in an acute care, rehab, or outpatient setting
Home health experience (2+ years preferred)
Strong clinical judgment and the ability to work independently
Reliable transportation to visit patients in their homes
