Home Health Physical Therapist (PT)

• $43.27 - $50.00 Hourly

Responsibilities:

• Monday – Friday – Flexible Schedule
• Coverage Areas: West St. Louis, Jefferson County, St. Charles County
• Evaluate patient needs and develop personalized treatment plans.
• EMR: Netsmart – OASIS Charting
• Provide therapeutic exercises and interventions to enhance function and quality of life.
• Educate patients and caregivers on safety, mobility techniques, and home exercises.
• Monitor progress and adjust care plans as needed.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.

Requirements:

• Active PT license in Missouri.
• Previous home health experience preferred.
• Strong communication and time management skills.
• Ability to work independently and travel locally.
