Home Health Physical Therapist (PT)

A Home Health Physical Therapist is needed in Aurora, Colorado.
Aurora is a thriving suburb of Denver offering a diverse community and outdoor lifestyle. This position is with a mission-driven home health organization focused on quality, patient-centered rehabilitation care.

Salary and Benefits

• $90,000.00 – $120,000.00 per year (pay-per-point model)

Responsibilities

• Provide in-home physical therapy to adult and geriatric patients
• Develop and implement treatment plans and document progress
• Collaborate with care team to support patient outcomes
• EMR: HomeCare Home Base
• Specialties include geriatrics, orthopedics, and rehabilitation
• Patient visits scheduled based on clinician availability
• 25 points weekly to meet productivity
• Start of care = 2 points
• All other visits = 1 point

Requirements

• Active Colorado Physical Therapist license (or ability to obtain)
• Degree in Physical Therapy from an accredited program
• Home health experience preferred
• Strong communication and time management skills
• Ability to work independently in the field
